Our study explored the impact of remotely delivered cognitive behavioral therapy on self-reported anxiety symptoms within the older adult community.
A meta-analysis and systematic review of randomized controlled trials, examining databases like PubMed, Embase, PsycInfo, and Cochrane until March 31, 2021, was carried out to determine whether remote CBT was superior to non-CBT control conditions in reducing self-reported anxiety in older adults. A standardized mean difference, using Cohen's d, was calculated for pre- and post-treatment values within each treatment group.
We calculated the effect size for cross-study comparison by contrasting the outcomes of the remote CBT group and the non-CBT control group, and then performed a random-effects meta-analysis. Self-reported anxiety symptoms, as measured by the Generalized Anxiety Disorder-7 item Scale, the Penn State Worry Questionnaire, or the abbreviated Penn State Worry Questionnaire, and self-reported depressive symptoms, assessed using the Patient Health Questionnaire-9 item Scale or Beck Depression Inventory, were respectively the primary and secondary outcomes.

This case report documents a 31-year-old Egyptian male's reaction to a 400mg intrathecal tranexamic acid injection, characterized by substantial back pain, gluteal pain, myoclonus in the lower limbs, agitation, and widespread convulsions, which followed a history of a left arm and right leg fracture. Immediate intravenous sedation with midazolam (5mg) and fentanyl (50mcg) proved ineffective in terminating the seizure. A 1000mg intravenous phenytoin infusion was given, followed by the induction of general anesthesia with the use of 250mg thiopental sodium and 50mg atracurium infusions. Subsequently, the patient's trachea was intubated. We examined peri-operative and post-operative outcomes, complications, and functional outcomes to differentiate between varying EEPs.
A systematic review and meta-analysis, adhering to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) checklist, was conducted. Only randomised controlled trials (RCTs) focused on comparisons between EEPs were incorporated. In order to assess risk of bias, the Cochrane tool for RCTs was used.
Following the search, 1153 articles were identified, and 12 RCTs were then chosen for inclusion in the analysis. A count of RCTs for each surgical technique comparison shows the following: 3 RCTs for HoLEP versus ThuLEP, 3 for HoLEP versus PKEP, 3 for PKEP versus DiLEP, 1 for HoLEP versus GreenVEP, 1 for HoLEP versus DiLEP, and 1 for ThuLEP versus PKEP. ThuLEP demonstrated reduced operative time and blood loss compared to both HoLEP and PKEP, while HoLEP exhibited faster operative time than PKEP. The blood loss associated with PKEP was greater than that associated with HoLEP and DiLEP. There were no Clavien-Dindo IV-V complications reported, and the incidence of Clavien-Dindo I complications was statistically lower in the ThuLEP group in comparison with the HoLEP group. Upon evaluating EEPs, no significant differences were noted with respect to urinary retention, stress urinary incontinence, bladder neck contracture, or urethral stricture.
