Surveys, qualitative interviews, administrative data, and medical records, including instances of MTPs and hemoglobin A1Cs, were part of the data sources collected during the initial three months of the one-year implementation period.
Qualitative interviews with six clinic liaisons, scrutinization of pharmacist observations, and open-ended survey questions aimed at clinic staff and providers, all contributed to the process of identifying lessons learned.
